Admission Policy

Applicants are admitted on the bases of the results of the entrance test, personal interviews, academic records, character evaluation, and the parents’ and pupil’s/student’s acceptance of SPCM’s Vision-Mission Statement, objectives, thrusts, policies, and procedures.

EARLY YEARS (Pre-Kinder, and Kindergarten)

 

· Application Form

· Philippine Statistics Authority (PSA) Birth Certificate (1 original & 2 photocopies)

· Parents’ Marriage Certificate / Affidavit of Marital Status / Solo Parent ID (original copy for verification & 1 photocopy)

· Baptismal Certificate (original for verification & 2 photocopies)

· 1”x 1” recent ID picture (2 pieces)

· Affidavit of Guardianship, if applicable (original copy for verification & 1 photocopy)

·  Custody related documents of a child whose parents are legally separated/have had their marriage annulled

· Health Certificate (form available online or at the Guidance Center) with updated
Immunization Record (form available at any Barangay Health Center)

· Updated assessment report and recommendation from Developmental Pediatrician or other
similar specialists (reading specialist, behavior therapist, occupational
therapist, etc) if the student-applicant has previously been diagnosed to have
a special needs/potent learning impediment. SPCM may not be equipped to cater
to the needs of differently-abled children.

 

Additional requirements for
student-applicants with previous schooling:

· Report Card (at least as of Second Quarter, 1 photocopy)

· Previous School’s DepEd Certificate of Recognition or Permit to Operate

· Filled out Recommendation Form with seal (by either Class Adviser or Guidance
Counselor in previous school), which are available online or at the Guidance
Center

 

 

GRADE SCHOOL / JUNIOR HIGH SCHOOL / SENIOR HIGH SCHOOL

· Application Form

·  Philippine Statistics Authority (PSA) Birth Certificate (1 original & 2 photocopies)

·  Report Card (at least as of Second Quarter, 1 Photocopy); with character grade of at least 83.00% or its 

equivalent letter grade, and with Learner’s Reference Number (original Form 138 will be

submitted upon enrollment with proper transfer credentials)

·  Parents’ Marriage Certificate / Affidavit of Marital Status / Solo Parent ID (original copy for verification

& 1 photocopy)

·  Baptismal Certificate (original for verification & 2 photocopies)

·  1” x 1” recent ID picture (2 pieces)

·  Affidavit of Guardianship, if applicable (original copy for verification & 1 photocopy)

·  Custody related documents of a child whose parents are legally separated/have had their

marriage annulled

·  Original Certificate of Good Moral Character issued by the previous school

·  Filled out Recommendation Form with seal (by either Class Adviser or Guidance Counselor in

previous school), which are available online or at the Guidance Center

·  Health Certificate (form available online or at the Guidance Center) with updated Immunization

Record (form available at any Barangay Health Center)

·  All incoming Grade 1 students must also submit clearance from a Developmental Pediatrician

to make sure the child receives early and necessary intervention. Student applicants

who have previously been diagnosed to have a special needs/potent learning impediment 

must submit updated reports from anyother similar specialists (reading

specialist, behavior therapist, occupational therapist, etc.) they are

currently working with. SPCM may not be equipped to cater to the needs of differently-abled 

children.

·  Certified True Copy of Result of the National Career Assessment Examination (for incoming 

students of Grades 10 and 11 levels)

FOREIGN APPLICANTS

·   All above-mentioned requirements (corresponding to the grade level being applying to)

·   Photocopy of Passport with at least 6 months validity

·   Photocopy of Alien Certificate of Registration (ACR) Card

·   Photocopy of Special Study Permit (SSP) from Bureau of Immigration and Deportation, if available (see Note a, below)

·   Transcript of Records/Permanent Record (see Note b, below)

 

Notes for Foreign Applicants:

a.    If student-applicant carries any of the following, he or she is excluded from submitting SSP:

·   Special Investment Resident Visa (SIRV)

·   Special Resident Retirees Visa (SRRV)

·   9G Visa

·   47(A)(2) Visa

·   9E (Diplomatic) Visa

b.    DepEd Regional Memorandum 2010

Accordingly, the scholastic record must be authenticated by the Office of the Philippine Embassy / Office of the Consular Official in the country which has jurisdiction over the issuing school, including an English translation of the entries particularly the name of student, subjects, grades and grade/year level. The translation will facilitate evaluation thereof.

 

 

  • Step 1

    Student-applicants must submit all required documents to the Guidance Center for evaluation. The submitted requirements will be assessed by a member of the Guidance Team, and the Registrar Staff.

  • Step 2

    Student-applicants will receive an Order of Payment slip, and must proceed to the Finance Office for the payment of the admission fee.

  • Step 3

    Student-applicants will forward the receipt of payment to the Guidance Center. Student-applicant will then be subject for preliminary interview and written exam (for Grades 3-11) to be conducted by his/her assigned level Facilitator-Counselor. The student-applicant’s parent will also be subject to a separate preliminary interview with the same Facilitator-Counselor. A Schedule of Admission Examination will be given through the Exam Permit.

  • Step 4

    Student-applicants will take the Admission Test on the scheduled date and time. The student-applicant and his/her parent will have their Final Interview with a member of the Academic Team.

  • Step 5

    Admission Results, duly signed by a Guidance Facilitator-Counselor and the Faculty member who conducted the Final Interview, will be issued to the student-applicant’s parent. Recipients of positive Admission Results may proceed for enrollment.

Notes for Admission Procedure:

  1. Contested cases of admission are decided on by the School Directress/Principal. Additional process-steps may be undertaken to better scrutinize student-applicant’s fitness for admission with the school.
  2. Parents of successful student-applicants must attend the ORIENTATION SEMINAR/PARENT ASSEMBLY, which shall be held before/right after the opening of the new academic year.

Mondays to Fridays

  • The Guidance Center is open from 08:00 AM to 12:00 NN, and 01:00 PM to 05:00 PM during weekdays.
  • The applicants may proceed for testing on the same day that they submit their requirements so long as the requirements submitted are complete.
  • The cut off time for testing is 10:00 AM for the morning schedule and 02:00 PM for the afternoon schedule.

Saturdays

  • The Guidance Center is open from 08:00 AM to 12:00 NN during Saturdays.
  • The applicants may proceed for testing on the same day that they submit their requirements so long as the requirements submitted are complete.
  • The cut off time for testing is 9:00 AM.
